The worst opening move is 1.g4 (the Grob), but there are openings that are worse since they make several bad moves, not just one. For example the Cow might be worse than the best Grob variation, if you play it with Black against some of White's best move orders.

The Damiano Defense is clearly the worst because white can open up the f-file with 3. Nxe5.
